Surgical Experiences on Acute Hemorrhagic Pancreatitis



Abstract
Supportive, non-operative therapy, has been found effective for most cases of cute pancreatitis.
However, conservative therapy has a low salvage rate in patients with fulminant hemorrhagic pancreatitis.
A group of 8 patients with severe pancreatitis was treated surgically from Jan. 1974, to Dec. 1975, in Baptist Hospital; Busan. The operative procedure consisted of peritoneal lavage, tube decompression of biliary and gastroinfestinal system by cholecystostomy, gastrosto my and jejunostomy.
Multiple intraperitoneal drains and sump drainage to pancreatic and retroperitoneal areas were placed.
Complications were sepsis, wound infection, pneumonia, intestinal obstruction, and pancreatic fistule.
One of eight expired from sepsis.
This preliminary report recommends operative intervention in patients with severe pancreatitis who deteriorate under maximal conservative therapy.
